Anything made from woods with knots like that will eventualy give a problem...usually in the form of a crack coming away from the knot...in an extreme case the knot could leave the wood.

The untrained eye sees knotty wood as something special the trained eye sees it for what it is....junk wood.

That's too bad that you feel that way about wood with such character.  Most woodturners who are worth anything at all have no problem dealing with knots, crotch wood or burls with No Ill effects. 
Ever hear of cyanoacrylate, "CA" (Super-Glue)?  Use it all the time and even use it as the actual finish on many a "Trash" Piece of wood.
 
Now Commander is right when he says that the knots could be a problem and many times can ruin a piece of otherwise spectacular wood - all I'm saying is that special care must be used when working with it.

I have never heard of "knotty osage". Unless knotty is being used as an adjective and not a part of the noun.

Most all the osage orange (hedgeapple)I have ever seen had knots. They make great bows. A little care and leaving a little extra wood around the knot seems to take care of knot issues. In bows anyway. I like the character knots add and consider those people that use it to usually be very fine craftsman. Sure it takes a little extra work but I like the look. Moisture is the main thing. If the moisture content is brought down slowly and no cracking has developed, I see no reason to expect any trouble out of a static piece of wood like that. Especially if it remains sealed and the humidity is somewhat steady. I doubt if there is a stronger, denser wood grown in the US as easy to come by.
Anyway, my 2 cents on wood.
