Remington Premier Magnum Question

I was patterning my 870 Super Mag at 30 yards using Remington Premier Magnum 3 oz. #5's.  It patterned nicely but I was greatly surprised to find one of the shots didn't spread out.  It left a 3 inch hole right where I would have wanted it to, but why wouldn't a random shot spread out?  Several other shells worked just fine.

My guess would be it had a bad wad that didn't open?

I had one that did the same thing last weekend but did open and let some pellets out.
